- I grant permission to allow my child to be met at Paterson Prep and walked to the Compostella After-School Program by a member of the Compostella staff or volunteers located on 24-26 DeGrasse St. Paterson, NJ 07505.
  
  Release of Liability: In consideration of the Cathedral of Saint John the Baptist accepting my/our son's /daughter's registration for the Compostella After School Program starting on September 16th, 2024. I/we release, hold harmless and discharge the Cathedral of Saint John the Baptist, Paterson, its officers, Trustees, employees, agents and affiliates, of and from any and all liabilit4-26 DeGrasse St. Paterson, NJ 07505.y, claim, loss, damage, cost or expense and waive any such claims against any such person or organization arising directly or indirectly from or attributable to any action or omission to act of any such person or organization in connection with the Compostella After School Program starting on September 16th, 2024 and I/we further agree to indemnify and hold harmless the Cathedral of Saint John the Baptist, Paterson and its aforesaid affiliated personnel from any such liability, claim, loss, damage, cost or expense.

- Yo doy permiso para permitir que el personal y los voluntarios de Compostella recojan a mi hijo/hija en Paterson Prep y lo caminen hasta el programa extraescolar de Compostella localizado en 24-26 DeGrasse St. Paterson, NJ 07505.
  
  Exoneración de Responsabilidad: En consideración a que la Catedral de San Juan Bautista acepta la inscripción de mi/nuestro hijo/hija para el Programa Compostella After School que comienza el 16 de septiembre de 2024. Yo/nosotros liberamos, eximimos de responsabilidad y descargamos a la Catedral de San Juan Bautista, Paterson, sus funcionarios, fideicomisarios, empleados, agentes y afiliados, de cualquier y toda responsabilidad, reclamo, pérdida, daño, costo o gasto y renuncio a dichos reclamos contra dicha persona u organización que surjan directa o indirectamente de o sean atribuibles a cualquier acción u omisión de actuar de dicha persona u organización en relación con el Programa Compostella After School que comienza el 16 de septiembre de 2024 y además acepto indemnizar y eximir de responsabilidad a la Catedral de San Juan Bautista, Paterson y su personal afiliado antes mencionado de cualquier responsabilidad, reclamo, pérdida, daño, costo o gasto.
